Day Pass Waiver

I acknowledge that participation in physical exercise and training activities at REP66 involves inherent risks of injury. I confirm that I am voluntarily participating in these activities with full knowledge of the risks involved.

I declare that I am physically fit and have no medical condition that would prevent me from safely participating in exercise, or I have disclosed any relevant conditions above and received clearance from my medical practitioner.

I release and hold harmless REP66, Brendan Burchall, and any associated staff from any claims, demands, or causes of action arising from my participation in training activities, except where caused by gross negligence.

I consent to receiving emergency medical treatment if required during my visit. I confirm that all information provided is true and correct.

This waiver is valid for the duration of my day pass visit to REP66.
